Type locality: Rio Chotano, between Chota and Cutervo, northern Peru, elevation 2350 m.  
TypesHolotype: MHNM (Museu Nacional de Historia Natural, Montevideo) CH 910. 
EtymologyNamed after Gerard Warden Teague (1885-1974), a systematic ichthyologist and herpetologist who was British Vice Consul for Paraguay.
